5.16 Termination of Employment-Procedures
Any faculty or academic staff member leaving the employment of the University must
have an Exit Procedure form completed and submitted to the Human Resources Office
before payment of the final salary check will be made. The form requires clearance from
the following areas: Physical Plant (keys), Textbook Library, Library Circulation Dept.,
Department Chair or Supervisor, Accounting Office (Procurement Card) and submission
of a forwarding address to the Human Resources Office. The Exit Procedure form can be
found on the UWRF HR website, under Forms.
