Falcon Athletic Training Forms

The following four forms should be printed, filled out completly and returned to the appropriate personnel by dates indicated.

No UWRF student-athlete will be able to practice until ALL forms are filled out and received.

Every student-athlete must also submit a copy of the front and back of their current insurance card.
